Note:
Death records on file in the City Clerk’s office date back to 1728. Records 50 years
old and older are public records. All records less than 50 years old are only
available to a close relative.
Where do I obtain a copy of a death certificate?
If the death
Occurred in Rhode Island after January 1, 2022
You may obtain the death certificate at the following locations:
- Any city or town in Rhode Island
- RI Department of Health, Vital Records
Division of Vital Records

Occurred prior to 2022 in Pawtucket
or
Occurred prior to 2022, a descendent resided in Pawtucket, and
the death occurred in the state.

What do I need to obtain a copy of a death certificate?
If arriving in person, present a picture identification of yourself.
If mailing a request, please fill out the following form:
What is the cost of a death certificate copy?
In person:
$22 for the first certificate
$18 for each additional certificate, of the same record, when
ordered simultaneously.
By mail or online:
$25 for the first certificate
$18 for each additional certificate, of the same record, when
ordered simultaneously.
Note:
Online requests will have an additional $5 surcharge when using a credit card.
